for the 2009 Wayne-Holmes Soap Box Derby. Information collected is confidential
and used only within the operations of the Wayne-Holmes Soap Box Derby.
You must enter and participate in the official Soap Box Derby CLOSEST to
your permanent residence per your Local area franchise agreement.
Submitting this form places the contestant on a mailing list of interested racers.
New entrants will be contacted by phone to determine final decisions on participation.

The above listed contestant hereby requests permission to enter and the above listed parent or guardian of such contestant hereby consents to such contestant entering the 2010 WAYNE-HOLMES SOAP BOX DERBY which is conducted under the terms of agreement between the ALL-AMERICAN SOAP BOX DERBY, and the WAYNE/HOLMES SOAP BOX DERBY, and in the event he/she is adjudged the winner of such Soap Box Derby, to his/her participating in the ALL-AMERICAN SOAP BOX DERBY at Akron, Ohio, during July 2010.

In consideration of the above listed contestant’s privilege of participating in such Soap Box Derbies, the above listed contestant and the above listed parent or guardian, jointly and severally, hereby (a) agree to be bound by such rules and regulations or changes therein as may be issued from time to time by the All- American Soap Box Derby, governing the conduct of the local Soap Box Derby and at the All-American Soap Box Derby, (b) acknowledge that the decisions of the officials at the local Soap Box Derby and at the All- American Soap Box Derby, as well as their interpretation of the rules and regulations, shall be binding upon all contestants, and (c) release the local sponsor(s) and the All-American Soap Box Derby from all liability which may arise or result out of, from or during the conduct of either the WAYNE/HOLMES SOAP BOX DERBY or the ALL-AMERICAN SOAP BOX DERBY. AND FURTHER AGREES THAT IF THIS CONTESTANT IS ADJUDGED WINNER OF THE 2010 WAYNE/HOLMES SOAP BOX DERBY HIS/HER WINNING RACER BECOMES THE PROPERTY OF THE WAYNE/HOLMES SOAP BOX DERBY ASSOCIATION.

By submitting this form the contestant agrees that he/she will build his/her own Soap Box Derby racer in accordance with the rules and regulations issued by the All-American Soap Box Derby & the Wayne-Holmes Soap Box Derby Association.
